Please complete this online form to register your child(ren) for Kol Ami Religious School. If you have more than one child to enroll, you can register them all at the same time using this online form.

You must be a parent or legal guardian of the child(ren) being registered to complete this form.

You will receive a confirmation email upon completion of this form. In late August, we will email you the school calendar, handbook, and instructions pertaining to the first day of school. These items will provide you with everything you will need to know about the start of the school year.

Our children and families come from many backgrounds. We are grateful both to Jewish parents and parents from other faiths who give us the gift of choosing to raise their children in the Jewish faith and making Kol Ami their spiritual home. We strive to do everything we can to support interfaith families raising Jewish children. While we understand that our children experience many cultures and traditions in their households and within their extended families, we ask that our parents choose one faith tradition for their child(ren)’s religious instruction and life-cycle rituals. This choice is essential in helping our children to develop a positive and unambiguous Jewish identity. We ask that our families who enroll their child(ren) at Kol Ami make the choice to raise their child(ren) exclusively as Jews and not enroll their child(ren) in formal religious instruction in another faith, or engage their children in religious life-cycle rituals outside of the Jewish tradition, either concurrently or sequentially with their Jewish education. If there is a special circumstance in your family or if you have any questions or concerns, please reach out to Rabbi Berkowitz so that we can work together to find the best way forward for your family. We invite parents, and grandparents, from all backgrounds to join in family learning experiences and participate in the life of the synagogue in ways that feel comfortable and appropriate. It’s a great way to learn more about Judaism, and it sets a wonderful example for our children to see their loved ones participating in Jewish life.

Student Walking to Religious School*

To keep our students safe, we need to know which child(ren) will be walking to and from Religious School both on Sunday and Wednesday. Your child’s safety is our immediate concern. We expect your child(ren) at school and if they do not show up, we will contact you or your emergency contact. On Wednesdays, please remind your walker child(ren) to walk directly to Kol Ami in order to arrive early to partake in snack time at 3:45 PM. This walker release effects all students in grades K-12.

From time to time photographs of the students are taken during school events and activities which Congregation Kol Ami may want to utilize in its print publications, online publications, presentations, website, and social media pages.

Permission Slip for Off-Campus Religious School Trips

Authorization for Off Campus Events: In the event of an off-campus excursion, I hereby give permission for the registered students to attend Religious School trip. I will be given ample communication prior to the event including topics, destination, departure and return times. There will be adults present during events and I will be provided with their cell phones for contact. For your information: The trip “lead” will carry a water proof and phone pack with a distribution list including contact information, medical treatment release, and the emergency contact information. First aid kits will be available, even if it is a nature hike to a nearby park. Parents or Emergency contacts will be called during the event if it is determined that the student is exhibiting inappropriate and unsafe behavior while out of the building and will need to pick up the student immediately. AUTHORIZATION TO TREAT MINOR: In the event that I cannot be reached in an emergency, I hereby permit a call to 911 and/or contacting a medical facility or physician selected by the trip leader to provide proper treatment to student and that I will be responsible for all expenses arising in association with such treatment. INDEMNITY AND WAIVER OF CLAIM: I, hereby acknowledge that as a condition of the student participating in these activities, agree to indemnify and hold harmless Congregation Kol Ami, its employees and volunteers, its governing board, the individual members thereof, and all other agents from any liability, lawsuit, cost, expense or claim of any type whatsoever (including legal fees) for any harm, injury or death arising out of the above mentioned activity.
